Job Purpose – overview

Your role as a Health and Wellbeing Consultant is to develop and deliver advanced health and exercise services to clients  

Core Accountabilities & Responsibilities

  • Develop and deliver exercise programs [based on the gold standard of level 4 qualifications] for people from within a clinical environment or a community setting with a number of long term health conditions
  • Tailor progressive exercise programmes appropriate to the medical condition presented using results from the physical/exercise assessments, medical information, national guidelines and client goals and objectives
  • Carry out assessments with families referred via Child Healthy Lifestyle/Get Going and where appropriate carry out coaching sessions and attend Get Going training
  • Audit exercise, health and lifestyle programmes provided by site-based Health & Wellbeing Advisors and provide guidance and support to all Health & Wellbeing Advisors
  • Develop positive relationships with health professionals
  • Monitor, evaluate and report on referral, retention and biometrical data analytics against any relevant agreed Key Performance Indicators.
  • Liaise with a variety of NHS teams, encouraging positive relationships and partnerships in order to continuously develop health and wellbeing within our community.
  • Provide internal blood pressure services and organise and deliver targeted events and courses in collaboration as and when required.
  • Manage tracking sheets on all referrals at each key stage, initial appointment & weeks 6,12,26,52 and 64
  • Keep up to date tracking sheets on referrals biometrical data
  • Maintain referral confidentiality in accordance with process guidelines, ensuring confidential records are stored securely
  • Deliver Health & Wellbeing appointments on site when possible to ensure all available time is maximised.
  • Willingness to continuously develop and attend training in line with your personal development plan, on occasions out-with your normal hours of work, to ensure the required level of competence

Essential Qualifications, Experience and Skills

Demonstrates positive core competencies

1 year level 3 gym instructor experience 

Level 3 GP Referral Qualification ie Wright Foundation or equivalent

Level 4 qualification eg. Mental Health (Level 4)   or  Cardiac Rehabilitation Phase IV (Level 4)

Hold or willingness to obtain and apply knowledge/skills from Emergency First Aid qualification.

Excellent interpersonal skills, including the ability to communicate professionally both verbally and in writing. 

An understanding of people with complex medical conditions and how to adapt specific activities/exercises to best meet their needs

Experience of delivering referral services to clients 

Knowledge of referral programmes

Hold and maintain a current valid driving license [and use of a vehicle].  

Desirable Qualifications, Experience and Skills

  • A Degree in Sport Science or exercise based discipline
  • 1 year experience in a similar post
  • Experience of partnership working with health professionals
  • Sports coaching qualifications
  • Routine maintenance of fitness equipment

Hold or willingness to achieve the following:

  • Pulmonary Rehabilitation Level 4
  • Exercise After Stroke Specialist Instructor Level 4
  • Exercise Based Rehabilitation in Cancer Care Level 4
